Required Participant Waiver

1. Permission to Participate

I, the undersigned, am the parent or legal guardian of the minor participant named above. I give my full permission for my child to participate in the youth fitness classes, strength training, conditioning, and related activities operated by Biscayne Barbell LLC (doing business as Biscayne Youth Fitness) held at Harris Field Gym in Homestead, FL, or any other designated training locations.

2. Assumption of Risk

I understand that participation in fitness and athletic activities involves inherent risks of physical injury. These risks include, but are not limited to, muscle strains, sprains, bruises, fractures, heat-related illnesses, or other injuries up to and including catastrophic injury or death. I knowingly and freely assume all such risks, both known and unknown, on behalf of my child. I certify that my child is physically fit and capable of safely participating in these activities.

3. Release of Liability

In consideration of my child’s participation, I hereby release, waive, discharge, and hold harmless Biscayne Barbell LLC, its owners, officers, employees, independent contractors, coaches, and agents, as well as the City of Homestead and Harris Field Gym staff, from any and all liability, claims, demands, or causes of action arising out of negligence, personal injury, property damage, or wrongful death occurring during or as a result of participation in the program.

4. Medical Emergency Authorization

In the event of an emergency, injury, or illness, I authorize the staff of Biscayne Barbell LLC to obtain necessary medical treatment for my child. This includes administering first aid, calling emergency services (911), and arranging transportation to the nearest medical facility. I accept full financial responsibility for any medical expenses incurred.

5. Media and Photo Release

I grant Biscayne Barbell LLC permission to photograph or video record my child during classes and to use these materials for marketing, website, and social media promotion.

6. Acknowledgment of Understanding

I have carefully read this waiver, fully understand its terms, and understand that I am giving up substantial legal rights, including the right to sue. I sign it freely and voluntarily.
